Please bring the following for admission to your interview:

• Last report card & current report card received from the previous school.

• All Assessments & Placement Test completely filled out (example FAS, SAT, FAIR testing scores).

• A copy of the student's health and shot records (blue & yellow form).

• A copy of student birth certificate

• A copy of student current IEP (if student is ESE program).

• Copy of student’s Psychological Evaluation.

• A copy of parent/guardian photo ID and SS Card 

• A copy of a utility bill in the parent name

• Copy of student’s Medicaid and/or Insurance Card

• $25 Non-refundable Registration & Technology fee​

• All information submitted will be kept by the Knowledge Builders Academy as part of the student's permanent file.
